Instructions for Application: The applicant should submit photocopies of the BIO page of the passport or international travel document, the endorsement page of a valid visa, and the page with the most recent entry stamp. If the entry stamp is on a previous passport or another international travel document, the applicant should additionally submit a photocopy of the BIO page of that passport or travel document.
