Instructions for Application: The original health examination records (including physical examination forms, laboratory test reports, etc.) must bear the official seal of the medical institution and be signed by a physician. The health examination records are valid for six months from the date of issuance. All documents must be in Chinese or English. Forms filled out in other languages will not be accepted. The overseas medical institution issuing the health examination records must be a public hospital designated by the Chinese consular authorities. Health examination certificates issued by private hospitals must be notarized by a local notary office.
