Qualifications 

1. Comply with the set-up plan of the local social welfare institutions; 

 

2. With fixed site for services;

 

3. The buildings shall comply with the requirements of "Code for Design of Buildings for Elderly Persons", "Code for Design of Buildings for the Convenience of the People with disability" and "Standards of Social Welfare Institution" of the Professional Standard of the People’s Republic of China;

 

4. With necessary facilities for daily life, medical care and rehabilitation, and with the space for indoor activities;

 

5. With the set-up funds in line with the content and scope of the services it offers; 

 

6. With completed management and service system; with the institution name complying with related regulation and requirement;

 

7. With professional management and service staffs in line with the services it offered.

Materials Required 

For examination and verification, the applicant shall submit: 

1. Application report; 

2. A feasibility study report;

3. Qualification certificate of the applicant;

4. Capital verification certificate of the applicant social institution;

5. Fixed site certificate of the applicant social institution;

6. Verification documents provided by Hunan Provincial Department of Commerce;

7. Agreement on joint venture and cooperation.

 

For acceptance inspection, the applicant shall submit: 

1. Application report in writing for the "Approval Certificate for Establishment of Social Welfare Institution"; 

2. Approval document for establishment of social welfare institution issued by the department of civil affairs;

3. Ownership certificate or lease contract of the service site;

4. Acceptance inspection report or examination opinions provided by the departments of construction, fire protection, health and immune;

5. Assets appraisal report;

6. Institution statue and completed management and service system;

7. Name list and the copies of effective certificate of the staffs on management, professional techniques and nursing, health certificate and pre-post training certificate of the working staffs;

8. Other related materials.

Procedures 

.Acceptance: 

1. Person in charge: staffs of the office of social welfare and social affairs in Department of Civil Affairs of Hunan Province

2.Responsibilities and Authorities: to checkup and verify the application materials according to "Interim Measures for the Management of Social Welfare Institution" and the regional planning for social welfare institutions; for the qualified case, accept it in time and report the verification opinions to the verification staff; for the unqualified case, refuse it and send back the materials to the applicant with explanation; inform the applicant in writing of the cases that need further materials and modification. 

 

. Examination: 

1. Person in charge: chief of the office of social welfare and social affairs in Department of Civil Affairs of Hunan Province. 

 

2. Responsibilities and Authorities: to examine the application materials according to related national laws and  regulations,  organize on-the-spot inspection and discussing meeting of the office; for the qualified cases, put forward examination opinions and report to deputy director-general in charge of the department; for the unqualified cases, sign for disapproval with explanations and submit to the accepting staffs to send back the materials to the applicant. 

 

. Decision: 

1. Person in charge: leader in charge of Department of Civil Affairs of Hunan Province 

2. Responsibilities and Authorities: for the approved cases, sign for the approval and submit to the office of social welfare and social affairs; for the unapproved cases, send back the application materials to the office of social welfare and social affairs, and give explanation in writing. 

 

. Informing and public notification 

1. Person in charge: staff of the office of social welfare and social affairs in Department of Civil Affairs of Hunan Province 

2. Responsibilities and Authorities: make licensing document and issue it to the applicant; inform the applicant that he or she has the right to apply for an administrative reconsideration or initiate an administrative lawsuit. 

Acceptable Period of Time 

Open for accepting applications all the year round; the service deadline is within 60 working days since the application is received.
